I authorize the reservation of an advertisement (size indicated above) in the 2012 edition of the Avery County Chamber of Commerce’s offical publication. I promise to pay the amount that coincides with the size of my ad indicated above to The Meridian Agency, the company contracted to publish the Guide by December 1, 2011. I understand that if my payment is not received by December 1, 2011, my ad will not be included in the Guide.
